The goal of the Royal Cardiac Care Management program is to improve the quality of life for our residents with congestive heart failure and cardiopulmonary disease. We identify the signs and symptoms of exacerbation before hospitalization is required while reducing hospital readmissions, enabling residents and families to make informed decisions regarding care and treatment and be active participants in managing care at home.

At Royal, discharge planning and education for short term care residents begins on the day of admission. Our team will help schedule follow-up appointments, and make sure the discharge summary is sent to both the resident’s primary physician and cardiologist.
